Osteoporosis and Frailty: What 2026 Research Reveals About Supplements

VITAMIN D AND FRAILTY: WHY STEADY BEATS MEGA-DOSES
Researchers Viola and colleagues explored how vitamin D acts across many body systems, supporting muscle strength, bone renewal, immune health, and energy production. (1) Low vitamin D levels are connected to muscle loss, weaker bones, and the onset of frailty. Yet, supplements appear to work best when someone is genuinely deficient or particularly vulnerable, and big, infrequent "mega-doses" have been associated with more falls and fractures. The takeaway: test, don't guess, and take vitamin D steadily as recommended.
CALCIUM AND VITAMIN D: A FOUNDATION FOR OSTEOPOROSIS CARE
A review by Pascual-Pastor and Barceló-Bru named daily calcium and vitamin D a cornerstone of osteoporosis care. (2) The authors reviewed the science on how the two nutrients work hand in hand to cut fracture risk and protect bone density, and noted that vitamin D deficiency is still widespread even among people who already have osteoporosis. They also highlighted the value of supplementation for people taking osteoporosis medications.
DO PROBIOTICS HELP BONE DENSITY?
Probiotics were a different story. In a carefully designed trial, Resciniti and colleagues gave 114 women in the first years after menopause either a daily probiotic or a placebo for a full year. (3) The probiotic did not slow bone loss, a helpful reminder that promising theories still need solid evidence, and that no single supplement replaces a thoughtful plan.
